Yes, it's Centropyge vroliki. This species hybridizes with other species (e.g., C. flavissima and C. eibli), and the hybrids sometimes turn up in the aquarium trade.
I got rid of it. Took it to my lfs. I acquired it with a tank purchase, and thought it was ugly. Plus it was aggressive towards the fish i like! I'm suspicious he was responsible for the death of my raccoon butterfly and convict tang.

Tangs and angels are very prone to it...Its a mystery disease ,,in the sense nobody really knows 100% why fish get it ,,or how to even treat it
In marine fish it is pretty well demonstrated that it is nutritional. Certain foods can prevent and reverse it and when the fish are placed in a very large, very mature system it can be reversed.
