Bury them deep, they are slow release. If any little balls of ferts do appear, re bury them

Depends partly on your tank and the brand. I was told every 4-6 weeks for mine.

As for the tank do you mean water changes? Or what the plants use or size?

Depends on your Plants, how many and the demand. Also how big your tank is and lots of factors. Swords are heavy root feeders. A big Amazon Sword will use a lot more ferts, than a tiny Chain Sword. A fast growing Stem plant may use more than a slow growing small Crypt.
